Ready to get started? It is real simple. Call us for an appointment: 281-466-8602 and we will determine your therapy needs and which counselor might best meet your needs. If you like, you can specify a counselor. Some of our counselors are in network providers for insurance; we can handle all out-of-network benefits. We can assist you with determining what your benefits are and with billing insurance. We will discuss your insurance with you prior to arriving for your first appointment. • Download our GPMFC informed consent form (PDF) if you are seeking individual, couple, or marriage counseling. If you are a couple or married each person needs to complete this form.
• Download the Adult 18+ Form (PDF) along with the consent form if you are adult. Again, if you are a couple or married each person needs to complete this form.
• Download the Child/Adolescent form (PDF) in addition to the informed consent form if you are bringing in a child/adolescent.
